Est\u00e0 a la copa d\u2019un roure i, malgrat aix\u00f2, t\u00e9 por d\u2019ofegar-se<\/span><\/em>\u00bb. (Cyrano de Bergerac).<\/p>\n<p>L\u2019afrontament integra processos atencional (an\u00e0lisis de la informaci\u00f3 entrant) que, com assenyala Morton (1969) i Neisser (1967) s\u00f3n facilitats per un an\u00e0lisi previ del context i les expectacions que intervenen en la situaci\u00f3.<\/p>\n<p>Els senyals sensorials, diu Norman (1969) al fil dels dos autors citats, prenen part en l\u2019activaci\u00f3 de les representacions guardades a la mem\u00f2ria. La selecci\u00f3 de material important es deriva de l\u2019impacte total de la senyal f\u00edsica que es produeix en aquell moment i de les expectacions i la pertin\u00e8ncia de les senyals possibles.<\/p>\n<p>Per tant, <span class=\"color_azul\">l\u2019atenci\u00f3 \u00e9s un proc\u00e9s general i un proc\u00e9s selectiu espec\u00edfic, encara que sembli un plantejament contradictori. De fet, no es pot donar un proc\u00e9s selectiu, que implica elecci\u00f3, si pr\u00e8viament tota la senyal entrant no rep un an\u00e0lisi de significat (proc\u00e9s general). A continuaci\u00f3, el mecanisme de l\u2019atenci\u00f3 selectiva assumeix les seves funcions, sempre sense \u00abdesconnectar\u00bb del proc\u00e9s educatiu general<\/span>.<\/p>\n<p>Aix\u00ed ho van posar de manifest experimentalment Treisman (1961) i Cherry (1954), citats per Norman (1969), seguint la t\u00e8cnica d\u2019investigaci\u00f3 que consisteix a mantenir el subjecte ocupat en una tasca de seguiment mentre que se li presenten diferents tipus de missatges i senyals que competeixen entre si. Aquests mateixos experiments proven que si el subjecte aconsegueix executar la tasca de seguiment, els grans canvis f\u00edsics del missatge no seguit s\u00f3n percebuts, els petits no i les paraules \u00abimportants\u00bb del missatge no seguit s\u00f3n percebuts freq\u00fcentment.<\/p>\n<p>Als efectes d\u2019aquest treball, ens interessa retenir tres q\u00fcestions molt generals:<\/p>\n<ul>\n<li><span class=\"color_azul\">l\u2019atenci\u00f3 guarda relaci\u00f3 amb significaci\u00f3 i avaluaci\u00f3 de l\u2019acci\u00f3 i l\u2019expectaci\u00f3<\/span><\/li>\n<li><span class=\"color_azul\">l\u2019atenci\u00f3 activa representacions guardades en la mem\u00f2ria<\/span><\/li>\n<li><span class=\"color_azul\">la funci\u00f3 selectiva de l\u2019atenci\u00f3 planteja algunes incompatibilitats<\/span><\/li>\n<\/ul>\n<p>Per al cas de l\u2019ansietat, l\u2019atenci\u00f3 autoenfocada \u00e9s condici\u00f3 necess\u00e0ria per a la hipervigil\u00e0ncia de les sensacions corporals relacionades amb l\u2019activaci\u00f3 psicobiol\u00f2gica.<\/p>\n<p>Wells (1991) en un estudi sobre la relaci\u00f3 entre atenci\u00f3 autoenfocada, avaluaci\u00f3 i ansietat, agafant com a subjectes a estudiants o graduats, va trobar que l\u2019autoobservaci\u00f3 venia aparellada amb l\u2019increment de l\u2019ansietat, nivells alts de preocupaci\u00f3 i s\u00edmptomes som\u00e0tics. Aquests tipus d\u2019experi\u00e8ncies s\u00f3n les que estableixen l\u2019autoenfocament. D\u2019altra banda, en un segon estudi, va utilitzar instruccions cognitives i som\u00e0tiques d\u2019autoatenci\u00f3 i instruccions de focus externs per verificar la hip\u00f2tesis que l\u2019atenci\u00f3 sobre la pr\u00f2pia activaci\u00f3 som\u00e0tica estava associada a l\u2019increment de l\u2019ansietat. Els resultats suggereixen que les tend\u00e8ncies espec\u00edfiques d\u2019autoenfocament estan associades a l\u2019elicitaci\u00f3 i exacerbaci\u00f3 de l\u2019ansietat. En resum, d\u2019una banda, \u00e9s m\u00e9s probable que es recorri a l\u2019atenci\u00f3 autoenfocada quan s\u2019experimenten nivells alts d\u2019ansietat i preocupaci\u00f3 acompanyats de s\u00edmptomes f\u00edsics intensos; de l\u2019altra, l\u2019atenci\u00f3 autoenfocada incrementa l\u2019ansietat.<\/p>\n<p>En un estudi comparatiu sobre la base de les hist\u00f2ries cl\u00edniques de pacients amb alteracions compatibles amb trastorns per ansietat, en el nostre centre de treball, hem trobat que l\u2019atenci\u00f3 autoenfocada \u00e9s significativament m\u00e9s freq\u00fcent en pacients que havien experimentat crisis d\u2019angoixa i en els que, en el context d\u2019ansietat generalitzada, no van relacionar les mol\u00e8sties f\u00edsiques amb cap problema personal racional. Per contra, algunes persones que origin\u00e0riament van relacionar la resposta emocional intensa amb circumst\u00e0ncies personals, o conflictes psicol\u00f2gics identificables considerats racionalment importants, no van mostrar una tend\u00e8ncia tan clara a autoobservar-se. \u00c9s com si quan es troba certa explicaci\u00f3 a l\u2019estat emocional, aquestes persones poguessin tolerar-ho millor i espantar-se menys, mentre que les altres, sorpresos per una reacci\u00f3 org\u00e0nica severa, potser dormint o en un moment de distensi\u00f3, o b\u00e9 en un context generalitzat de tensi\u00f3 inespec\u00edfica, entenen que alguna cosa nova i\/o greu, org\u00e0nicament parlant, ha comen\u00e7at a passar en el seu organisme.<\/p>\n<p>De fet, <span class=\"color_azul\">alguns autors, Clark (1988) entre ells, han postulat i han tractat de demostrar experimentalment que les crisis d\u2019angoixa depenen que el subjecte interpreti les seves pr\u00f2pies sensacions corporals d\u2019una manera particular<\/span>, de manera catastrofista. Aquesta \u00e9s la tesi central del model cognitivista del p\u00e0nic (Beck i Emery (1985), i Barlow (1988)).<\/p>\n<p>Segons Clark, el manteniment del trastorn, quan l\u2019individu ja ha desenvolupat una tend\u00e8ncia a interpretar catastr\u00f2ficament les sensacions corporals, ve donat per <span class=\"color_azul\">dos processos addicionals: l\u2019atenci\u00f3 autoenfocada que porta a l\u2019individu a ser m\u00e9s conscient de les seves sensacions i la evitaci\u00f3 de situacions o activitats que comporten intensificaci\u00f3 de les sensacions<\/span>, permet que el pacient mantingui un estil interpretatiu negatiu, ja que no permet verificar la racionalitat de les seves pors a si mateix (Ballester, Botella, Gil i Ferrer, 1991).<\/p>\n<p>Cal afegir, en aquest sentit, que la disminuci\u00f3 de l\u2019ansietat quan la persona, acompanyada, s\u2019exposa a la situaci\u00f3 temuda, t\u00e9 a veure, en alguns casos, amb que redueixi la seva actitud hipervigilant i preventiva, ja que considera que l\u2019acompanyant o b\u00e9 l\u2019ajudar\u00e0 a afrontar all\u00f2 que sol \u00e9s incapa\u00e7, o b\u00e9 l\u2019atendr\u00e0, en cas necessari, fet que redueix la necessitat de prevenci\u00f3; mentre que en l\u2019exposici\u00f3 en solitari es tem la possibilitat d\u2019experimentar angoixa i perdre la capacitat de reacci\u00f3, cosa que porta a exercir m\u00e9s prevenci\u00f3, autoobservaci\u00f3 i autopron\u00f2stic. A vegades, el que preocupa al pacient, en principi, no s\u00f3n les manifestacions de l\u2019ansietat, sin\u00f3 el lloc on es poden desencadenar i quines possibilitats d\u2019escapat\u00f2ria tindria. L\u2019agoraf\u00f2bia (por al metro, a l\u2019autob\u00fas, retencions, a les aglomeracions, a l\u2019ascensor, etc.) deu part de la seva intensitat i capacitat angoixant, no nom\u00e9s a possibles processos de condicionament cl\u00e0ssic, sin\u00f3 tamb\u00e9 als processos preventius del pacient ansi\u00f3s que va pensant qu\u00e8 passaria \u00absi li agaf\u00e9s l\u2019atac\u00bb en totes i cadascuna de les situacions i circumst\u00e0ncies possibles o necess\u00e0ries. En aquelles situacions en qu\u00e8 es pogu\u00e9s veure sense sortida, l\u2019ansietat que es provoca \u00e9s considerablement m\u00e9s alta, fet que a la vegada consolida l\u2019establiment de la f\u00f2bia.<\/p>\n<p><span class=\"color_azul\">En un altre ordre de coses, cal assenyalar que l\u2019atenci\u00f3 sobre els s\u00edmptomes f\u00edsics de l\u2019ansietat, normalment dolorosos, incrementa l\u2019impacte del dolor i dificulta l\u2019habituaci\u00f3 subjectiva (Arntz, Dreessen i Merekelbach, 1991).<\/span><\/p>\n<p><span class=\"color_azul\">Cal plantejar-se la pregunta de fins a quin punt l\u2019atenci\u00f3 autoenfocada interfereix sobre l\u2019execuci\u00f3 de les tasques.<\/span> Rich i Woolever (1988) van examinar els efectes de l\u2019expectativa de resultats, atenci\u00f3 autoenfocada i ansietat amb relaci\u00f3 a l\u2019activitat, en estudiants universitaris. 40 subjectes que van obtenir una puntuaci\u00f3 alta en un test d\u2019ansietat i 40 que van obtenir una puntuaci\u00f3 baixa, van rebre un <em>feedback<\/em> anterior d\u2019\u00e8xit (expectativa positiva) o frac\u00e0s (expectativa negativa) en una tasca d\u2019associaci\u00f3 de paraules que es va presentar com a predictiva d\u2019activitat en tasques d\u2019adquisici\u00f3 verbal. La meitat dels subjectes van completar les tasques d\u2019adquisici\u00f3 subseg\u00fcent en pres\u00e8ncia d\u2019un mirall per induir l\u2019atenci\u00f3 autoenfocada. Entre els subjectes amb ansietat (resultats d\u2019ansietat alta en els tests) amb atenci\u00f3 autoenfocada, es va donar una facilitaci\u00f3 de l\u2019activitat significativa, sota condicions d\u2019expectativa positiva i es va produir una disminuci\u00f3 significativa, sota condicions d\u2019expectativa negativa. \u00c9s arriscat extrapolar an\u00e0lisi per a la cl\u00ednica procedents d\u2019estudis amb subjectes no cl\u00ednics, per\u00f2 poden tenir un valor indicatiu.<\/p>\n<p><b>M\u00e9s informaci\u00f3<\/b><\/p>\n<p>ARNTZ, A., DREESSEN, L., MERCKELBACH, H.
