{"id":56058,"date":"2021-10-19T12:16:39","date_gmt":"2021-10-19T10:16:39","guid":{"rendered":"https:\/\/clinicadeansiedad.com\/uncategorized-ca\/fobias-de-impulsion\/"},"modified":"2024-02-19T12:56:07","modified_gmt":"2024-02-19T12:56:07","slug":"fobias-de-impulsion","status":"publish","type":"post","link":"https:\/\/clinicadeansiedad.com\/ca\/problemes\/fobias-de-impulsion\/fobias-de-impulsion\/","title":{"rendered":"F\u00f2bies d&#8217;impulsi\u00f3."},"content":{"rendered":"\n<p>Les <strong>f\u00f2bies d&#8217;impulsi\u00f3<\/strong> son un tipus caracter\u00edstic <strong>d&#8217;obsessi\u00f3 <\/strong>en el que els <strong>pensaments, imatges o impulsos intrusos<\/strong> tracten sobre fer-se mal a un mateix, als altres, o tenir algun comportament molt inapropiat, amb conseq\u00fc\u00e8ncies greus o irreversibles.<\/p>\n\n\n\n<p>Exemples habituals i molt freq\u00fcents entre els nostres pacients son clavar-li un ganivet a alg\u00fa, llen\u00e7ar-se pel balc\u00f3 o finestra, donar-li una bufetada o ferir a una persona estimada, emp\u00e8nyer a alg\u00fa al metro o llen\u00e7ar-se, insultar o agredir sobtadament a alg\u00fa&#8230;etc.<\/p>\n\n\n\n<p><strong>La persona, al tenir aquets pensaments, imatges o impulsos creu que \u00e9s probable perdre el control<\/strong>. Aquesta por, i l&#8217;ansietat que genera, li fa pensar que ho pot arribar a fer, o que s&#8217;est\u00e0 tornant boig, o que no \u00e9s gens normal que aix\u00f2 li passi, en definitiva, que quelcom no funciona be en ella.<\/p>\n\n\n\n<p>Les obsessions, son pensaments intrusius, que apareixen de manera no desitjada (s\u00f3n involuntaris) i que es repeteixen constantment. El contingut, en el cas espec\u00edfic de la f\u00f2bia d&#8217;impulsi\u00f3, \u00e9s amena\u00e7ant o moralment reprovable, com hem comentat en els exemples anteriors.<\/p>\n\n\n\n<p>Aquestes obsessions poden presentar-se com:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Imatges: <\/strong>la persona es veu a si mateixa llan\u00e7ant-se per la finestra o empenyent a alg\u00fa, o insultant a crits a algun sense motiu, etc. S&#8217; imagina aquesta escena, les conseq\u00fc\u00e8ncies que tindria, les expressions de por de les persones que estan davant..etc.<\/li><li><strong>Pensaments<\/strong>: la persona creu que far\u00e0 alguna d&#8217;aquestes coses encara que o ho vegi en imatges. Li poden apar\u00e8ixer pensaments \u201cimperatius\u201d del tipus: \u201cdona-li un cop de puny\u201d, o \u201cllen\u00e7a&#8217;t pel balc\u00f3\u201d. Pensa en lo terrible que ser\u00eda fer-ho, que dirien els altres, com reaccionarien..etc.<\/li><li><strong>Impulsos<\/strong>: la persona t\u00e9 la sensaci\u00f3 de que ho far\u00e0 Sent l&#8217;impuls i creu que arribar\u00e0 a fer-ho irremeiablement.<\/li><\/ul>\n\n\n\n<p>En moltes ocasions aquestes obsessions apareixen no nom\u00e9s d&#8217;una manera, si no que de manera combinada. Potser primer ve una imatge i despr\u00e9s pensaments al respecta, o primer se sent l&#8217;impuls i despr\u00e9s venen tot un seguit d&#8217;imatges al cap.<\/p>\n\n\n\n<p>Quan aix\u00f2 passa, la persona experimenta un <strong>gran malestar i ansietat<\/strong>, i intenta evitar-ho. Per una banda, al creure que finalment podria passar o que no \u00e9s gens normal pensar aquestes coses, evita estar en contacte amb situacions que ho facilitarien: procura no tenir ganivets a la vista, o no fa servir els que son grans, o els guarda r\u00e0pidament quan els veu; potser no s&#8217;apropa al balc\u00f3 i encara menys si porta el seu fill a coll; o s&#8217;atura lluny del pas de vianants si la obsessi\u00f3 \u00e9s llan\u00e7ar-se i ser atropellat. Tracta de reduir la possibilitat de que passi o neutralitzar l&#8217;ansietat que li produeixen aquestes obsessions.<\/p>\n\n\n\n<p>Al creure que tenir aquest tipus de pensaments, imatges o impulsos no \u00e9s normal, a m\u00e9s d&#8217;evitar situacions, llocs o objectes, comen\u00e7a a sentir-se molt malament amb elles mateixes, i a criticar-se i menystenir-se: \u201ccom pot ser que pensi tot aix\u00f2\u201d, \u201cestic malament del cap\u201d, \u201cuna persona normal no pensa tot aix\u00f2\u201d. A m\u00e9s, creuen que el fet de pensar-ho \u00e9s un indici de que ho faran o podrien fer-ho, que podrien perdre el control i finalment fer-ho. Molts pacients expressen aquesta preocupaci\u00f3 dient \u201c\u00e9s que nom\u00e9s que per un moment se me\u2019n vagi el cap&#8230;\u201d o \u201c\u00e9s tan f\u00e0cil que passi\u201d.<\/p>\n\n\n\n<p>Algunes de les pors principals associades a la f\u00f2bia d&#8217;impulsi\u00f3 s\u00f3n:<\/p>\n\n\n\n<p><em><strong>(\u00c9s important assenyalar que totes aquestes pors son creences falses).<\/strong><\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>M&#8217;estic tornant boig o boja.<\/li><li>\u00c9s molt f\u00e0cil perdre el control.<\/li><li>Si ho penso tant finalment ho far\u00e9.<\/li><li>Soc una persona horrible per pensar totes aquestes coses.<\/li><\/ul>\n\n\n\n<p><strong>Qu\u00e8 treballem a la ter\u00e0pia per solucionar les f\u00f2bies d&#8217;impulsi\u00f3?<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>La persona apr\u00e8n que \u00e9s un pensament obsessiu, que \u00e9s la f\u00f2bia d&#8217;impulsi\u00f3, i per tant pot entendre millor el que li passa.<\/li><li>S&#8217;expliquen, per cada cas particular, les creences falses associades a les seves pors.<\/li><li>La persona apr\u00e8n a reinterpretar que significa tenir aquets pensaments, imatges o impulsos.<\/li><li>S\u2019aconsegueix manegar-les millor quan apareixen, i una notable reducci\u00f3.<\/li><li>La persona aconsegueix eliminar l&#8217;ansietat en relaci\u00f3 a la f\u00f2bia d&#8217;impulsi\u00f3.<\/li><li>Poc a poc la persona aconsegueix afrontar les situacions que evitava i tornar a la seva vida amb normalitat, sense estar condicionada.<\/li><\/ul>\n\n\n\n<p>\u00c9s important assenyalar que per aconseguir-ho fa falta temps i treball conjunt entre psic\u00f2leg\/a i pacient.
