{"id":55291,"date":"2015-09-23T03:31:07","date_gmt":"2015-09-23T03:31:07","guid":{"rendered":"https:\/\/clinicadeansiedad.com\/uncategorized-ca\/agorafobia-origen-y-mantenimiento\/"},"modified":"2024-02-19T12:57:31","modified_gmt":"2024-02-19T12:57:31","slug":"agorafobia-origen-y-mantenimiento","status":"publish","type":"post","link":"https:\/\/clinicadeansiedad.com\/ca\/problemes\/crisis-de-panico-y-agorafobia\/agorafobia-origen-y-mantenimiento\/","title":{"rendered":"L\u2019agoraf\u00f2bia: L\u2019origen i el manteniment"},"content":{"rendered":"<p><span class=\"tit_azul\">L\u2019adquisici\u00f3 de la por de les situacions agoraf\u00f2biques<\/span><\/p>\n<p>Hi ha diversos factors que interactuen o que sumen les seves influ\u00e8ncies, tot i que \u00e9s dif\u00edcil d\u2019establir qu\u00e8 n\u2019\u00e9s una causa, qu\u00e8 n\u2019\u00e9s una conseq\u00fc\u00e8ncia o quan existeix una relaci\u00f3 entre variables.<\/p>\n<p>Entre &nbsp;elsfactors que s\u2019han estudiat s\u00f3n les experi\u00e8ncies traum\u00e0tiques directes fora de casa, com atacs de p\u00e0nic, desmais, asf\u00edxiales; les caracter\u00edstiques de la personalitat, l\u2019atribuci\u00f3 err\u00f2nia de sensacions , &nbsp;o la vulnerabilitat gen\u00e8tica com a factor de rellev\u00e0ncia.<\/p>\n<p><span class=\"tit_azul\">Els esdeveniments estressants<\/span><\/p>\n<p>La gran majoria de les persones amb agoraf\u00f2bia informen d\u2019haver passat per un per\u00edode d\u2019estr\u00e8s o per esdeveniments vitals importants en l\u2019\u00e8poca en qu\u00e8 va comen\u00e7ar l\u2019agoraf\u00f2bia: conflictes interpersonals amb el marit o amb algun altre familiar, un divorci o una separaci\u00f3, un naixement o un avortament, la malaltia o la defunci\u00f3 d\u2019una persona significativa, una reacci\u00f3 al consum de drogues, malalties o operacions importants, estr\u00e8s laboral o escolar, o problemes econ\u00f2mics, entre d\u2019altres.<\/p>\n<p>L\u2019estr\u00e8s pot originar f\u00f2bies nom\u00e9s en persones particularment vulnerables. Aquesta vulnerabilitat \u00e9s probable que sigui el resultat de la interacci\u00f3 entre les variables biol\u00f2giques, les ambientals i les psicol\u00f2giques.<\/p>\n<p><span class=\"tit_azul\">Les caracter\u00edstiques personals i les experi\u00e8ncies durant la inf\u00e0ncia<\/span><\/p>\n<p>\u00c9s possible trobar una s\u00e8rie de caracter\u00edstiques en les persones que pateixen agoraf\u00f2bia que poden contribuir al desenvolupament d\u2019aquest trastorn. Algunes d\u2019elles s\u00f3n la susceptibilitat a l\u2019ansietat, l\u2019assertivitat i la confian\u00e7a en ells mateixos escasses, l\u2019ansietat i el retra\u00efment social elevats, la por de l\u2019avaluaci\u00f3 negativa, l\u2019alta recerca d\u2019aprovaci\u00f3, estrat\u00e8gies ineficaces per afrontar l\u2019estr\u00e8s, la tend\u00e8ncia a l\u2019evitaci\u00f3 dels problemes interpersonals, l\u2019ansietat tret elevada, el neuroticisme elevat, l\u2019elevada sensibilitat al dany, l\u2019elevada introversi\u00f3 o la inhibici\u00f3 conductual davant de coses desconegudes.<\/p>\n<p>\u00c9s possible que el fet de viure algunes experi\u00e8ncies durant la inf\u00e0ncia estigui relacionat amb l\u2019agoraf\u00f2bia: una mare o un pare sobreprotectors o mancats d\u2019afecte poden interferir en el desenvolupament de la independ\u00e8ncia i del sentiment de compet\u00e8ncia del nen. Pot ser que l\u2019ansietat per separaci\u00f3 en la inf\u00e0ncia estigui relacionada amb el desenvolupament d\u2019un trastorn de p\u00e0nic en la vida adulta i que l\u2019ab\u00fas sexual i el maltractament f\u00edsic a edats primerenques de la vida siguin factors de risc per desenvolupar diversos trastorns psicopatol\u00f2gics.<\/p>\n<p><span class=\"tit_azul\">L\u2019activaci\u00f3 fisiol\u00f2gica i les sensacions corporals<\/span><\/p>\n<p>A m\u00e9s dels factors comentats anteriorment (les situacions temudes, els esdeveniments estressants i l\u2019ansietat elevada), hi ha altres factors que poden produir o facilitar l\u2019aparici\u00f3 de sensacions corporals, com ara:<\/p>\n<ol>\n<li>Els problemes m\u00e8dics endocrins, cardiovasculars, respiratoris o neurol\u00f2gics.<\/li>\n<li>Els factors biol\u00f2gics com els canvis hormonals o la predisposici\u00f3 per tenir sensacions corporals.<\/li>\n<li>Les imatges i els pensaments negatius amb relaci\u00f3 a patir un atac de p\u00e0nic, a la mort o a sofrir un desmai.<\/li>\n<li>Altres factors que poden facilitar o provocar l\u2019activaci\u00f3 fisiol\u00f2gica i les reaccions corporals com l\u2019\u00fas excessiu de cafe\u00efna, de nicotina, d\u2019alcohol o d\u2019altres drogues o f\u00e0rmacs, l\u2019excitaci\u00f3, la ira, la fatiga, la falta de son, l\u2019exercici, els canvis posturals r\u00e0pids o la calor.<\/li>\n<li>La hiperventilaci\u00f3.<\/li>\n<\/ol>\n<p>La hipervigil\u00e0ncia o atenci\u00f3 centrada en els elements amena\u00e7ants de les situacions temudes i en determinades sensacions corporals associades a l\u2019ansietat, pot facilitar l\u2019ocurr\u00e8ncia d\u2019episodis d\u2019ansietat o de p\u00e0nic, ja que es perceben les sensacions subjectivament de manera magnificada i augmenta l\u2019activaci\u00f3 fisiol\u00f2gica.<\/p>\n<p><span class=\"tit_azul\">La interpretaci\u00f3 de les sensacions com a senyals de perill, d\u2019ansietat o de p\u00e0nic<\/span><\/p>\n<p>Els subjectes poden pensar que les sensacions corporals presagien o tindran conseq\u00fc\u00e8ncies perilloses o catastr\u00f2fiques (un atac de cor, un desmai, la mort, una incapacitat, una p\u00e8rdua de control) o que s\u00f3n senyals que avisen que aquestes conseq\u00fc\u00e8ncies ja estan passant.<\/p>\n<p>La percepci\u00f3 t\u00e9 un paper molt important en aquest tipus de trastorns. La percepci\u00f3 dels canvis fisiol\u00f2gics, s\u2019ajusti o no a la realitat, acostuma a caracteritzar-se per ser m\u00e9s precisa en els subjectes amb atacs de p\u00e0nic i agoraf\u00f2bia. Aquests subjectes s\u00f3n capa\u00e7os de detectar canvis fisiol\u00f2gics molt subtils, tot i que normalment no en s\u00f3n conscients.<\/p>\n<p>La sensaci\u00f3 de perill provocada per l\u2019activaci\u00f3 fisiol\u00f2gica produeix un increment addicional d\u2019aquesta activaci\u00f3 mitjan\u00e7ant la hiperventilaci\u00f3 o altres mecanismes, de manera que es tanca un cercle vici\u00f3s que pot provocar un episodi d\u2019ansietat o un atac de p\u00e0nic.<\/p>\n<p>Pot arribar a establir-se una associaci\u00f3 directa entre determinades sensacions corporals i l\u2019ansietat o el p\u00e0nic (condicionament interoceptiu).<\/p>\n<p><span class=\"tit_azul\">La fugida i l\u2019evitaci\u00f3 de les situacions temudes<\/span><\/p>\n<p>L\u2019anticipaci\u00f3 o l\u2019ocurr\u00e8ncia d\u2019un atac d\u2019ansietat o de p\u00e0nic, els pensaments catastr\u00f2fics i el desig d\u2019evitar la vergonya social, fan m\u00e9s probable que el subjecte fugi de la situaci\u00f3. Hi ha dues maneres de fugir-ne: marxar de la situaci\u00f3 temuda o romandre en ella fent la fugida desviant l\u2019atenci\u00f3 cap a altres est\u00edmuls.<\/p>\n<p>Els agoraf\u00f2bics utilitzen una gran varietat d\u2019estrat\u00e8gies, a banda de la fugida i la distracci\u00f3, per afrontar els atacs de p\u00e0nic: medicar-se, beure alcohol, aguantar, relaxaci\u00f3, pensar que l\u2019atac passar\u00e0 aviat, estirar-se, respiraci\u00f3, imatges agradables, fumar, etc.<\/p>\n<p>La conducta de fugida es veu refor\u00e7ada per la reducci\u00f3 de l\u2019ansietat, per la sensaci\u00f3 de seguretat i fins i tot per l\u2019atenci\u00f3 i pel suport dels altres.<\/p>\n<p>L\u2019ansietat anticipat\u00f2ria apareix davant la possibilitat d\u2019entrar en una situaci\u00f3 temuda o de patir alguna conseq\u00fc\u00e8ncia temuda, cosa que facilita la hipervigil\u00e0ncia de determinades sensacions corporals, fet que, a la vegada, pot afavorir detectar certes sensacions, incrementar l\u2019activaci\u00f3 fisiol\u00f2gica i produir por o ansietat.<\/p>\n<p>Hi ha persones que desenvolupen un trastorn de p\u00e0nic per\u00f2 sense agoraf\u00f2bia; \u00e9s a dir, no presenten l\u2019evitaci\u00f3 agoraf\u00f2bica i mantenen una actitud d\u2019afrontament davant les dificultats. Aquestes persones tenen una sensaci\u00f3 de control personal i una toler\u00e0ncia a l\u2019ansietat m\u00e9s grans, i obtenen menys beneficis de la conducta d\u2019evitaci\u00f3 (suport social, reducci\u00f3 de l\u2019ansietat, etc.).<\/p>\n<p>La conducta d\u2019evitaci\u00f3 acostuma a tenir conseq\u00fc\u00e8ncies negatives, com ara la reducci\u00f3 dels contactes socials, la depressi\u00f3, la baixa autoestima, la interfer\u00e8ncia amb les activitats d\u2019oci, la disfunci\u00f3 sexual o problemes laborals, maritals i familiars.<\/p>\n<p>__________<br \/>\n<span class=\"color_azul\">Font<\/span>: E.
